In the majority of cultures and societies gender is strongly defined as male and female. Anyone that does not feel they fit in one of these definitions is presented with life problems. The social consequences of being gay, transgender, or anything else than the standard has proven to be enormous. In most countries it means being socially excluded from education, work, health care and family life.
To turn this around, a new definition is needed, that allows for diversity in society. Focus on single issues as only HIV / STD problems, rarely help to bring about the changes needed to resolve the social consequences.
On the other hand we see an upcoming definition of blaming society and naming people who don’t like gender issues by any means as being backward and ridicules and even right winged fascist.
Freedom of being we feel is a tension to deal with. Not a choice for any point of view.
